Certain SonoHealth devices require a prescription or a healthcare provider's recommendation before they ship. This one-minute form is the fastest way to complete verification: upload a photo of your prescription or provider recommendation, or give us your provider's contact details and we'll confirm with their office for you.

Your privacy: this form is provided by HIPAAtizer, an encrypted third-party service. Your information is used only to verify your order, is never sold or shared, and is not stored on our store platform. Most verifications complete within one business day — your order ships the same day verification completes. If we can't verify within 5 business days, we cancel and refund your order in full.
